Description: This packet includes three different assessments: Comprehensive assessment, Short assessment, Acute Care assessment. Each assessment can be adapted for different settings and ages to help determine the treatment plan.
The comprehensive assessment includes seven pages of guided statements of client behaviors, responses, preferences, etc. Statements are split between different goal areas and each area also includes one blank to add statements that are not listed on the form.
The short assessment assessment form is a one-page document with fill-in and check-list structure. The form includes sections for client needs, strengths, musical responses, and other comments.
The acute care assessment form will help you gather pertinent information for an in-the-moment assessment by using a Likert scale for pre and post ratings.
Depending on the business, facility, and therapist preference, the forms can serve as the sole assessment document or accompany an additional written report. Each of these assessments may also be used in tandem with our "Music Therapy Intake Form" under the documentation section of our website.
